Description of Our Services

Greater Heights Health Care provide a full range of domiciliary/home care services, that actively support individuals to remain in their own homes and communities for as long as it is safe and appropriate for them to do so. Greater Heights Healthcare is registered with Care Inspectorate Wales (CIW) to provide Domiciliary Care Services within Newport and the Gwent Areas.

Our services are available to all vulnerable adults, including those within ethnic minority communities; providing staff that are skilled and competent in meeting the identified needs of individuals in various ethnic groups, i.e. linguistically, religiously, dietary and so on

Greater Heights Health Care boasts of a team of trained and competant staff suitable to provide care and support to vulnerable adults in accordance to the agreed care plan. Support provided may include all activities of the day such as:

Greater Heights Health Care will always ensure that each individual service user is placed at the center of the planning of their care and support and that they are treated with dignity and respect at all times, while maintaining and promoting each individuals’ rights, privacy and dignity.

Our services are available to all vulnerable adults, through a variety of funding routes, such as, Local Authority, Healthcare Trusts, private means, direct payments, individual budgets, joint and/or collaborative funding.

We provide our care/support services 365 days a year and are able to provide care from half an hour to 24 hours a day, dependent on the individuals identified and agreed needs. All costs/charges are calculated on the identified individual need and are charged one month in arrears.

The Needs of our Customers

Greater Heights Health Care provides an outcome focused, person centered service to each individual service user that we provide care/support to. Needs are identified using the Local Authority (or PCT) care plan and carrying out an individual needs assessment with each service user prior to the commencement of service, from which an individual care/support plan will be agreed and drawn up that will be used as a working document to meet those identified needs of the individual. These needs may be, but not limited to:

 

● Personal care needs – washing, oral hygiene, bathing, dressing
● Dietary needs – meal planning, preparation, feeding
● Laundry – ensuring clean, dry and appropriate clothing and bedding is readily available to the service user
● Light housework – carrying out vacuuming, dusting, washing dishes, changing/making beds
● Medication needs – assisting to manage prescribed medication
● Shopping and correct storage of foodstuffs
● Participation in community/social activities
● Attending medical/out-patient appointments
● General companionship

 

Greater Heights Health Care staff will at all times maintain accurate written records in line with the individuals agreed care/support plan, that will contain information in regard to the support offered/provided and the uptake of this by the service user at each call.

How we satisfy our Customers’ Needs

Customer satisfaction is our aim, and we endeavor to do this by ensuring compliance with legislative standards at all times, developing and agreeing an outcome focused, person centered care/support plan that is regularly reviewed and updated as and when required by individual need.

Working closely with members of the multi-disciplinary team and ensuring that each individual service user and/or their representative is fully informed and aware of the role and responsibilities of the organisation and its staff is paramount to ensuring that individual needs are met.

Providing all of our customers with clear information in regard to the practices, policies and procedures of the organisation, allows for informed choices to be made in regard to the provision of service.

Responding promptly, effectively and professionally to all concerns and/or complaints will ensure that service provision continually meets service user need and legislative requirements. Positive and effective communication with all relative parties and on-going quality assurance processes will also be implemented to ensure that customer needs are satisfied and responded to in an appropriate manner at all times.

Staff matching will be carried out for each individual service user and core teams of staff assigned to each individual care package; transparency and openness throughout the organisation will reassure customers that we are at all times focused on their needs and identified outcomes.

Greater Heights Health Care will always provide a cost-effective service that meets the identified needs of each service user.

How our Customers’ Needs may Change

Greater Heights Health Care recognize that individual needs and wants are ever changing and are fluid dependent on the health and well-being of that individual at any specific time, to this end Greater Heights Health Care ensure that all care/support plans, risk assessments, behaviour plans and so on are reviewed and updated on a minimum of a three-monthly basis.

By providing core staff that provide continuity of care Greater Heights Health Care can and will respond promptly and effectively to an individual changing need(s) and can amend care/support packages in order that need(s) are continually met in a person centered and cost effective manner.

The ability of Greater Heights Health Care to respond flexible to changing needs is key to providing a quality service, increasing and/or decreasing levels of support, facilitating additional assessment and/or professional reviews, recognizing achievements and so on reflects Greater Heights Health Care’s proactive approach to responding to changing individual service user need.

Greater Heights Healthcare to understand the importance of positive response to the on-going global pandemic of Covid19. Greater Heights HealthCare will respond promptly, positively, and effectively in not only protecting those who use our services but also our staff team, families, and the wider community.
